Listed below are some of the major plans in which we participate. We urge you to check with your insurance company to ensure we are participants in your plan. If we are participants in your plan, we will gladly file your insurance on your behalf. If we are not participants in your plan, we ask for payment at time of service. Aetna (HMO/POS/PPO) *Not all doctors are on all plans. Please contact the office to verify that your doctor is on your plan. ATTENTION MEDICARE PATIENTS: Our physicians have “opted out” of Medicare. This status means that we are not allowed to file claims with Medicare and the patient is not allowed to file for reimbursement from Medicare. We will be unable to file your secondary insurance claims as we are unable to file your primary insurance claim. We will be unable to provide claim forms as your account will not be set up for insurance filing. We are happy to see Medicare patients as “self pay” patients, but we require payment in full at the time of service. As with all “self pay” patients (those patients without insurance accepted by our office), we are pleased to be able to offer a 30% discount on the office visit and on bone density studies. This discount does not apply to procedures or ancillary charges such as PapSures, urinanalysis, pregnancy tests, injections, hemoglobin, etc., but only to the charge for the office visit. We will continue to review our status with Medicare every year to determine if we will remain in the “opt out” status. |
